Referral form

The Referral form to the Home should be included the following details:

• Applicant’s personal information;
• family background;
• behavioral assessment;
• physical/mental health assessment, drug prescriptions or special care needs;
• long-term welfare plan.

HCG will set up an interview with the applicant and her parents/guardian once an application received. If the application is accepted, enrollment to the Home will be processed once consent is given by the applicant and her parents/guardian.
